Our History
FOREST CITY DIAGNOSTIC IMAGING opened in 1998. We were formally known as Forest City Open MRI. We expanded our business in January 2005 and moved to our current location. Our services now include not only an Open MRI, but we've added a state of the art Hi-Field MRI. Our latest addition of services is X-Ray, CT Scans and PET/CT Scans. We are the only provider of a fixed site PET/CT machine in Northern Illinois (not brought in via semi-trailer like the hospitals). You are now able to access this service 5 days a week.

Our Facility
Our facility has the feel and comfort of home to help you feel more at ease. We can also assist with the insurance paperwork associated with your healthcare. Forest City DI participates with almost all insurance companies, including BCBS, Humana, ECOH, PHCS, Workmans’ Compensation, Medicare and Medicaid. Filmless imaging from MERGE Technologies is the leading edge technology that we use.

Our doctors read all our Diagnostic studies on computer monitors. In addition your physician is able to access your images via our website. We will also provide images on compact disks (CD's) for viewing on computer monitors.

Childcare
If you have young children, make arrangements for a baby-sitter or child care, before your exam date. Children are not allowed in the examination room and may not be left in the waiting area unattended.

Our Hours
We have convenient hours:
Mon, Wed, and Thurs 7:00 am - 10:00 pm
Tuesday and Friday 7:00 am - 5:00 pm
Saturday 7:00 am - 2:00 pm
*Additional hours upon request.

We can often scan patients the same day the scan is ordered. A final report is available for the ordering physician within hours.

Appointment Changes and Test Results
1. If you must cancel or reschedule your appointment, please call the receptionist at (815)398-1300. Please call at least 24 hours in advance.

2. After the film is developed and read by our Radiologist, a copy of the results are sent to your physician. Only your physician can give you the results of your test.

3. The Radiologist bills separately for his services. You will receive two bills for the procedure-one from Forest City Diagnostic Imaging and one from the radiologist for reading the films. The Radiologist are members of Rockford Radiology Associates.

4. You will be given a copy of your images on a CD as part of your personal records. Your doctor will let us know what method he prefers to access your images. We take care of the rest.
